C&W discovered the following evidence to help support the denial off a new loss saving our client from having to pay out on an orthopedic claim.

The insured employer reported that during filming in Peru, the claimant sustained an injury. C&W discovered a news story that indicated otherwise.

Based on data secured early by C&W, a public school employee was convicted of insurance fraud and was ordered to pay back $15,090.00 in restitution along with a C&R and resignation from her employment.

During C&W’s investigation, we discovered anomalies that were turned over to the FBI and California DOI Fraud Investigators to help in the apprehension of the claimant and her co-conspirators.

From data secured early by C&W, a public employee was found to have secondary employment doing the same work they claimed they were unable to preform due to an alleged work related injury. Based on this, surveillance captured footage to validate the activity, and secondary employment records were secured. The claimant is now looking at criminal charges for insurance fraud. It also turns out that the injured worker was not paying taxes for the side business and additional charges were filed for tax evasion.

In 2019, C&W was requested to conduct a background / social media investigation on an evidence technician who claimed to have received an alleged respiratory injury due to toxic exposure from bags of marijuana. They claimed that over the course of a few years, marijuana was improperly bagged and stored causing moldy conditions. Our initial investigation found the claimant active, and engaging in secondary employment.

Fast forward to 2024 and the claim is in mediation. The claimant’s attorney requested vocational rehabilitation for his client as the form of a buyout that could cost the client anywhere between $20,000 to $80,000 depending on the claimants’ wages.  C&W was once again called upon and pulled over 80 additional pages of social media activity showing the claimant still actively selling real-estate and maintaining an active lifestyle.

For an investment of $425, our client was able to save upwards of $80,000 on this one claim.

In July of 2023, C&W was requested to conduct a background / social media search for a client. The claimant reported a left shoulder injury with a date of loss in May of 2023. We found the claimant to be active on social media, and possibly working. Photos were secured of the claimant holding a fish for a photo that exceeded stated limitations and restrictions. We also preserved photos of the claimant working at a gym, possibly as a personal trainer. We noted the following posted in June of 2023 to social media: “Tore my shoulder at work according to MRI … I can’t lift my shoulder over my head without pain, I can’t reach out to grab anything without horrible pain. I haven’t been able to work out.”

Based on C&W’s results, employment records were obtained as well as other supporting evidence to include surveillance video, and the claim was ultimately denied.
